RESEARCH COMMUNICATION Risk Factors for Post-ERCP Cholangitis in Patients with Pancreatic Cancer from a Single Referral Center in Iran

چکیده
Cholangitis is relatively uncommon (Boey & Way, 1980). Any condition that leads to stasis or obstruction of bile in the common bile duct (CBD), including benign or malignant stricture, parasitic infection, or extrinsic compression by the pancreas, can result in bacterial infection and cholangitis (Lam et al., 1978; Boey & Way, 1980). Choledocholithiasis is the most common cause of acute cholangitis, followed by endoscopic retrograde cholangiopancreatography [ERCP], and tumors. After ERCP 1-3% of patients develop cholangitis (O’Connor et al., 1982a: 1982b). Obstruction of the bile duct by tumor or stones can facilitate bacterial colonization; subsequent instrumentation has resulted in bacteremia rates mean 18.0% (O’Connor et al., 1982a: 1982b; Gerecht et al., 1989). The highest bacteremia rates are seen in therapeutic ERCP (Gerecht et al., 1989). In purely diagnostic ERCP, the bacteremic rate is lower at 8% (O’Connor et al., 1982b; Gerecht et al., 1989). Partial obstruction is associated with a higher rate of infection than complete obstruction (Boey & Way, 1980; Lam et al., 1978). Risk of cholangitis is increased if dye is injected retrograde (Boey & Way, 1980). Mortality of cholangitis is high due to the predisposition

Risk factors for post-ERCP cholangitis in patients with pancreatic cancer from a single referral center in Iran.
Cholangitis is relatively uncommon but associated mortality is high due to the predisposition in people with underlying disease. For this recognition of contributing risk factors is necessary. Therefore, the present descriptive- analytical cross-sectional survey was designed to determine contributing risk factors for post-ERCP cholangitis in patients with pancreatic cancer. From 2005 to 2010, 1...
متن کاملIdentification of risk factors for post-endoscopic retrograde cholangiopancreatography pancreatitis in a high volume center
BACKGROUND/OBJECTIVES Pancreatitis is the most common complication of endoscopic retrograde cholangiopancreatography (ERCP). Several patients´ or procedure related risk factors for post-ERCP pancreatitis (PEP) have been suggested. The aim of this study was to validate the risk factors for PEP in a high-volume center. METHODS All patients undergoing first time ERCP at a tertiary referral cente...
متن کاملRisk factors for therapeutic ERCP-related complications: an analysis of 2,715 cases performed by a single endoscopist
BACKGROUND Endoscopic retrograde cholangiopancreatography (ERCP) is now the exclusive endoscopic therapeutic modality for biliary as well as pancreatic diseases. The aim of the present study was to investigate patient- and procedure-related risk factors for post-ERCP complications in a large-scale study of procedures performed by a single experienced endoscopist. METHODS This is a retrospecti...
متن کاملThoracoscopic Splanchnicectomy for Pain Control in Irresectable Pancreatic Cancer
Introduction : Severepain is a major problem in patients with unresectable pancreatic cancer. The goal of this study is to evaluate the effects of Thoracoscopic Splanchnicectomy (TS) on pain control in these patients suffering from unresectable pancreatic cancer. Methods:Between years 2000 to 2011, 20 patients suffering from unresectable pancreatic cancer underwent TS due to severe pain. They w...
متن کاملPost-ERCP acute pancreatitis and its risk factors
INTRODUCTION Endoscopic retrograde cholangiopancreatography (ERCP) is a complex endoscopic technique that evolved from a diagnostic to a mainly therapeutic procedure. This was due to the identification of post-procedural complications that can follow both simple ERCP and that associated with the instrumentation of the biliary and pancreatic ductals. The identification of post ERCP complications...
